Client initiatives that have excelled with the FTI approach.
FTI partnered with The Bizzell Group to develop three Opioid Management Change Packages for Ambulatory Care Settings. FTI interviewed more than 30 health care practices across the country to identify effective practices around opioid management using evidence-based guidelines and demonstrated results, for managing opioid pain and administering effective treatment programs for opioid use disorder (OUD). FTI synthesized and created guiding principles, strategies and actions from high performing ambulatory sites to serve as the national guidance for all ambulatory care sites to adapt and implement. The three change packages developed together serve as a community guide for the management of pain, opioid use and opioid use disorder (OUD).
FTI supported the Health Services Advisory Group with the development of two change packages for the national kidney donor and transplant communities. FTI led interviews with over 200 individuals from high performing donor hospitals, kidney transplant facilities and organ procurement organizations across the country to surface best practices for increasing deceased donor kidney transplants and decreasing the discard rate of procured kidneys. FTI synthesized best practices into two change that will serve as the guiding principles for the upcoming CMS ESRD Treatment Choices (ETC) Kidney Transplant Learning Collaborative.
The Alliance for Integrated Medication Management (AIMM) is a non-profit organization mobilizing national and local partners to build healthier communities through safe and effective medication. AIMM initiatives support wide-spread adoption of comprehensive medication management services into the care of high-risk, high-cost patients suffering from multiple chronic health conditions. FTI administers AIMM’s collaborative programs in which community partnerships establish innovative health care delivery systems around medication management services. FTI has designed learning experiences that have enabled more than 400 health care organizations to strategically plan, to facilitate practice changes, and to support value-based payment models that include medication management services.
Transforming Clinical Practice Initiative
Supported under the Centers for Medicare and Medicaid Services (CMS), the Transforming Clinical Practice Initiative (TCPI) is one of the largest federal investments uniquely designed to support clinician practices through nationwide, collaborative, and peer-based learning networks that facilitate large-scale practice transformation. FTI is responsible for developing and executing the strategy to transform more than 140,000 clinicians and prepare them to be successful in a value-based payment environment. FTI developed and executed the performance system that describes progress towards the achievement of the project aims and serves as improvement coaches offering guidance designed to coach, mentor and assist clinicians in developing core competencies specific to practice transformation.
Partnership for Patients
Launched by CMS, The Partnership for Patients initiative is a public-private partnership with a focus on reducing preventable patient harm such as hospital acquired infections and avoidable readmissions, in accordance with best practices for hospitals. FTI served on the national content development team, responsible for implementing the action learning collaborative approach used to drive performance across the national partnership and the awarded Hospital Engagement Networks. FTI led the restructuring on behalf of CMS to the overall PfP program in 2012 and has remained involved since, through technical support and guiding the affinity groups on identified deliverables such as adverse drug events and readmissions reduction.
Community-based Cares Transition Program
The CMS supported Community-based Care Transitions Program (CCTP) tested models for improving care transitions from the hospital to other settings and reducing readmissions for high-risk Medicare beneficiaries. FTI oversaw the process for developing the change package, an evidence-based set of changes that are critical to the improvement of an identified care process, designed the learning sessions that have the community teams in action learning, coached the staff in the effective leadership style and created a management information system model to show sets of process and outcome data that successful site teams use to manage the program.
